Q. How do I use my vision benefit?

Using your Physicians Eyecare Plan vision plan is easy. Locate an in-network provider in your area by clicking here.

Call the provider to schedule your appointment. Let the provider know that you are a Physicians Eyecare Plan member at the time you schedule your appointment.

Please be sure to show your ID card at the time of your appointment. The in-network provider’s office will file all claims. You are responsible for payment to the in-network provider for any amount exceeding the material allowance and any copays.

Q. How do I find an in-network provider?

You can find an in-network provider by using the form to the right or by clicking here.

Q. Do I have to go to an in-network provider?

You may go to any doctor you choose. You will receive the most benefit from your Physicians Eyecare Plan vision plan by seeing an in-network provider but you may go to any doctor you choose. For more information on out-of-network benefits and how to file an out of network claim please click here.

Q. How do I submit a claim for reimbursement?

You only need to submit a claim for reimbursement if you see a provider other than an in-network provider. For more information on out-of-network benefits and how to file an out-of-network claim please click here.

Q. What is the difference between a routine eye exam and a contact lens exam?

Contact lens exams are designed to evaluate your vision with contacts. Your vision benefit does not cover the contact lens portion of your exam. However, we have negotiated reduced pricing for our members when using an in-network provider.
