Out-of-network claims

Submit an out-of-network claim if you have seen an out-of-network provider or utilized an in-store sale or promotion from an in-network provider.

If you choose to use an out-of-network provider or utilize an in-store sale or promotion from an in-network provider, pay the provider in full for the services and eyewear received at the time of your appointment.

You can download a claim form for submission via mail or fax. Or submit a claim online!

You will receive a reimbursement check within 5 weeks of when your claim was submitted.

Visit our Find A Provider page and search for providers by zip code or address. Choose the provider you wish to see from the list and call to schedule your appointment. Let the provider know that you are a PEP Vision member at the time you schedule your appointment.

Please be sure to show your ID card at the time of your appointment. The in-network provider’s office will file all claims. You are only responsible for payment to the in-network provider for any amount exceeding your material allowance and any copays.

No. You will receive the most benefit from your vision plan by seeing an in-network provider, but you may go to any provider you choose. For more information on out-of-network benefits and how to file an out-of-network claim, please click here.

You only need to submit a claim for reimbursement if you see a provider that is not an in-network provider or if you purchased products or services through a sale or promotion from an in-network provider. Yes. You can view your benefits, request an ID card, and view existing claims by logging into the Member Portal.   If you have any issues logging into the Member Portal, call us at 1-800-368-9609 Monday – Friday 9:00 a.m. to 5:00 p.m. (EST) to have a Member Service representative assist you.

Yes. You can print an ID card and request an ID card be mailed to you by logging into the Member Portal.  If you have any issues logging into the Member Portal, call us at 1-800-368-9609 Monday – Friday 9:00 a.m. to 5:00 p.m. (EST) to have a Member Service representative assist you.

Yes. You can purchase prescription glasses and contact lenses online. This would be considered an out-of-network claim. Out-of-network claims may be processed at a reduced benefit as per your benefit summary. Please refer to your out-of-network benefits to determine if this would be processed at a reduced benefit.

No. PEP Vision does not coordinate benefits with other vision plans or vision benefits under health insurance plans.
